As an experienced Mental Health Pharmacist, you will be providing mental health services with regular review of medication charts, medicines reconciliation on admission and discharge, ensuring safe, effective and patient-centred medicines decisions are made.

*Assessment of medicines management standards within our hospital organisations and associated care facilities
*Ensure our hospitals meet CQC regulations and best practice requirements, offering advice and solutions where necessary to deliver a safer and more effective medication management system
*Review of medication prescription charts and statutory MHA paperwork to ensure compliance with the MHA and assessment of clinical appropriateness of prescribing
*Promote the safest and most effective treatment plans through shared knowledge and expertise
*Attendance at MDT meetings and ward round participation with consultant psychiatrists, nurses and other HCPs
*To provide pharmaceutical medication one-to-one reviews and health education advice to patients during the pharmacist hospital visits
*Deliver training to doctors, nurses and healthcare assistants working for our hospital clients to help ensure compliance with continual professional development
Expenses: mileage is paid for and travel expenses are reimbursed
In order to qualify for this position, it is essential that you an MPharms Degree and are GPhC registered, as well as having substantial post registration experience as a Mental Health Pharmacist.
